Brief Description:
Holstep Health are seeking an Aged Care Project Officer to join the team.

- This is an exiting opportunity to work collaboratively across Holstep Healths aged care programs
- Permanent, Part Time position
- Site: Various Holstep Health Sites (primarily from: West Heidelberg and Chiefly Drive locations)

YOUR NEW ROLE:
Holstep Health are seeking an experienced and motivated Aged Care Project Officer for the implementation and delivery of systemic changes to support quality aged care services and programs at Holstep Health, including Holstep Health's Support At Home (SAH) Transition.

Working collaboratively with internal teams and stakeholders, this role will contribute to project coordination, service integration, quality improvement and strategic initiatives that enhance outcomes for older people within the community.

This is an exciting opportunity for someone with strong project management, strategic and operational leadership, and organisational skills who is passionate about improving aged care service delivery within a community health setting.

YOU WILL BE RESPONSIBLE FOR:
As an Aged Care Project Officer and reporting to the Director, Aged &Primary Care, you will be responsible for:

• Analysing reform initiative and external policy implication to provide advice to aged care management and staff at Holstep Health
• Lead the ongoing review and implementation of Holstep Health's Aged Care Transition Plan
• Manage and deliver sub-projects within the Holstep Health's Aged Care Transition Plan.
• Bring and maintain strategic knowledge of the aged care reforms timelines, agendas and key priorities to inform Holstep Health's planning and change activities.
• Develop in-depth understanding of Holstep Health's aged care services and programs to effectively inform system change and transition planning and activities.
• Develop response to industry wide consultation in line with Holstep Health's strategic position.

WHAT YOU NEED TO SUCCEED:
We are looking for individuals with:
• Qualifications in health information, statistics, epidemiology/social research, data science or public health discipline.
• Proficient understanding of the Community Aged Care sector
• Demonstrated capability to work in an environment of uncertainty
• Demonstrated change management leadership
• Conceptual and analytical skills to inform decision making
• Ability to build and foster relationships with internal and external stakeholders.
• Experience in project management and coordination
• Highly developed communication skills, including capability to develop to inform industry consultation papers
• Organisational and interpersonal skills, e.g. negotiation, problem solving capacity, communication and organisational and planning skills
